Registered Nurse - Hospice

Job Description

Job Summary:

Provide compassionate hospice nursing care to patients in their homes, nursing facilities, or group homes. Lead interdisciplinary teams to ensure comprehensive, patient-centered support and symptom management.

Responsibilities:

• Assess patient needs through thorough clinical evaluations and develop individualized care plans.
• Educate patients and caregivers on disease processes, symptom management, and care goals.
• Administer treatments, medications, and interventions according to physician orders and best practices.
• Collaborate with interdisciplinary teams to coordinate care and ensure seamless communication.
• Document patient progress, care activities, and clinical findings accurately and timely.
• Provide emotional support and comfort to patients and their families during challenging times.

Qualifications:

• Valid RN license with hospice care experience required.
• Proven ability to conduct comprehensive assessments and develop care plans.
• Strong clinical skills in symptom management and patient education.
• Ability to work independently and as part of a multidisciplinary team.
• Excellent communication, critical thinking, and problem-solving skills.
• Flexibility to participate in rotating on-call and occasional weekend shifts.
