Role & Responsibilities

• Compassionate End-of-Life Care: Provide sensitive, resident-centric daily personal care to patients in their home settings, ensuring comfort, dignity, and peace.
• Patient Hygiene & Comfort: Assist with personal hygiene, grooming, bathing, light nutrition, and repositioning/safe transfers. Measure and record vital signs.
• Family Support & Communication: Serve as a comforting, professional presence, reporting any clinical changes or urgent needs promptly to the hospice nursing supervisor.
• Environment Maintenance: Assist with maintaining a clean, calm, and supportive home environment for the patient and their visiting loved ones.
• Technology Proficiency: Demonstrated ability to navigate MatrixCare efficiently, ensuring timely and accurate documentation of shift start times, travel, and patient care details.
• Ability to Travel: Capable of independent travel to various local client residences, maintaining detailed and precise records of travel time and destinations.

Requirements

• Must possess an active Pennsylvania CNA certification
• Minimum 6 months of hospice, home health, skilled-nursing, or related clinical experience
• Highly punctual, reliable, and trustworthy
• A deeply caring, empathetic, and resilient attitude suited for home hospice environments
• A reliable vehicle for travel between patient homes
