Description

• Assess patients in inpatient and outpatient settings, creating personalized treatment approaches
• Implement physical therapy interventions that enhance mobility, strength, balance, and overall functionality
• Work collaboratively alongside nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes
• Provide education to patients and their families about therapeutic goals, discharge plans, and at-home exercises
• Maintain accurate documentation of evaluations, progress, and provided treatments as per facility and regulatory standards
• Engage in multidisciplinary team rounds and conferences to facilitate integrated care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure required as a Physical Therapist
• CPR certification preferred but not mandatory
• Additional specialized certifications (e.g., neurological or geriatric) are a plus but not required

Skills

• Experience in a similar role is highly preferred
• Skilled in navigating patient complexities and mobility challenges
