Disability Services Consultant - Bachelor's Required
Provident Behavioral Health
Seeking an IRIS Consultant to support individuals in maintaining independence and navigating long-term care services. Responsibilities Include Building trusting relationships with participants Developing IRIS Support and Services Plans Maintaining regular contact and conducting visits Explaining IRIS eligibility and connecting with local agencies Documenting interactions accurately and timely Potentially conducting annual functional screens Collaborating with a supportive team while working independently Qualifications And Skills Bachelor’s degree in social work, psychology, or related field with at least one year of supervised experience working with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing Bilingual skills a plus Strong commitment to participant autonomy and empathy Proficiency with smartphones, tablets, and computers Knowledge of human behavior, social resources, and community systems Willingness to serve diverse populations with respect Requirements Valid driver’s license and reliable transportation Satisfactory driving record and auto insurance compliance This role offers benefits such as: loan forgiveness support health insurance paid time off retirement contributions wellness resources recognition programs #J-18808-Ljbffr Provident Behavioral Health
