Compassionate Hospice RN Case Manager

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patient-centered care in home settings across Texas. You will conduct comprehensive assessments, develop and adjust plans of care, coordinate with the IDT, and educate families about end-of-life care. The role emphasizes compassionate, compliant nursing, with on-call participation and strong documentation practices.
