RESPONSIBLE TO: Pharmacy Manager
SALARY: Step 53($135,917) - ($238,332); Full Benefits

POSITION OBJECTIVES
The Clinical Staff Pharmacist I will collaborate with primary care providers and staff to provide high quality pharmacy services and patient care. Duties include ensuring appropriate drug selection considering clinical, formulary and reimbursement factors; prescription final verification; medication dispensing in person or by delivery; oversight of inventory management; providing mediation therapy management (MTMs); following and assisting with protocol and guideline updates; drug use evaluations; administrative assistance and patient education & encounters (counseling/clinical interventions). The Clinical Staff Pharmacist I will also continually work to develop patient care programs that will serve the best interest of our patients. The Clinical Staff Pharmacist I will strive to improve patient care and service for individuals eligible for services provided by Klamath Tribal Health & Family Services in homes, schools, clinics, alternate job sites and other community locations within the Klamath Tribes’ Service Unit.
MAJOR DUTIES AND RESPONSIBILITIES
1.Serve as a team member with the pharmacy staff to maintain and develop pharmacy services and as a drug information center to provide high quality patient care.
2.Maintain current knowledge of and comply with applicable state and federal laws, rules and regulations, along with Pharmacy Department contractual requirements, policies and procedures.
3.Ensure accurate preparation and dispensing of pharmaceuticals for patient care according to professional standards along with legal, contractual, policy and procedural requirements.
4.Ensure appropriateness of drug therapy in light of clinical, formulary and cost-effective care when checking prescriptions, reviewing profiles, managing the medication synchronization program, and other patient care opportunities.
5.Contact and confer with the prescriber in a timely manner to clarify prescriptions & answer questions regarding drug/product selection to ensure appropriate patient care.
6.Provide patient counseling, verifying and assuring that patients understand their medication, appropriate outcomes of their drug therapy, other self-care education for their treatment plan and when to seek follow-up care.
7.Provide drug information and drug therapy consultation to patients and the health care team when approach and to address clinical or other identified opportunities to improve patient care within the pharmacy.
8.Appropriately use reference materials to better inform and guide patients, providers and others about specific drug therapy options.
9.Maintain established inventory procedures for counting, record keeping, dispensing, securing and completing monthly count and reconciliation of controlled substances; safely handling hazardous drugs; and assuring availability of acute and chronic use cost-effective medications.
10.Assist with directing workflow, managing ancillary staff and ensuring prescription delivery is timely and accurate. Stepping in to provide all types of pharmacy operational support and delegating responsibilities as appropriate.
11.Support high quality patient care through weekly compliance and individual bubble packs, the medication synchronization program, the delivery program and other unique services regularly provided for patient care.
12.Use Electronic Health Records suitably to document patient care through abstracting medication, medication reconciliation, updating allergies or intolerances in coordination with the primary care team, sending verbal orders on behalf of the provider and assuring demographics are accurate in coordination with the patient registration team.
Ensure appropriate documentation within the pharmacy system including patient or provider specific information, updating allergies to match the Electronic Health Record (email notification), documenting patient preferences, clinical or legal notations on specific prescriptions, etc.
14.Assist in the orientation, training and education of support personnel, pharmacist, pharmacy technicians, clerks and pharmacy students.
15.Make informed rational and timely decisions. Exercise independent judgment within the limits of authority. Consult with appropriate personnel in matters of uncertainty by discussing formulary compliance, order entry and dosing of medications when applicable.
16.Self-identify professional areas to develop knowledge and skills for the pharmacy practice as they arise through literature review, continuing education and collegial sharing of information and changing professional practice to provide the highest level of pharmaceutical care.
17.Influence others to strive towards organization and departmental objectives by serving as a role model demonstrating a contemporary attitude & focusing on patient care outcomes.
18.Assist with interdisciplinary committees, drug protocol reviews, medication use evaluations, standard operating procedure (SOPs) updates and other clinical and administrative activities as needed.
19.Provide health promotion and disease prevention activities relating to drug use and preventive drug therapy.
20.Receive training specific to the detection, correction and prevention of fraud, waste and abuse in the delivery of Medicare and Medicaid benefits. Training shall be performed within 30 days of hire and at least annually thereafter.
21.Like all employees of the Klamath Tribes, the incumbent will be called upon to accomplish other tasks that may not be directly related to this position, but are integral to the Klamath Tribes’ broader functions, including but not limited to, assisting during Tribal sponsored cultural, traditional, or community events that enable the successful operation of programs and practices of The Klamath Tribes as aligned with The Klamath Tribes’ Mission Statement. Some of these tasks may be scheduled outside of regular work hours, if necessary.
SUPERVISORY CONTROLS
The Clinical Staff Pharmacist I works under the direct supervision of the Pharmacy Manager who provides administrative program direction. Work is assigned in terms of functional/organizational objectives. The supervisor will provide guidance with unusual situations that do not have clear precedents.
The employee plans and carries out the day-to-day activities independently, resolves problems on the basis of past precedent; exercises judgment in interpreting guidelines and applicability; adhering to professional practices and legal requirements.
The employee is subjected to a variety of situations that would require using independent judgment and discretion. Assignments are reviewed for quality and compliance with established policies and procedures.
KNOWLEDGE, SKILLS, ABILITIES
Possess working knowledge of Federal, State, and contractual laws, regulations, rules and professional standards regarding the practice of pharmacy.
Ability to perform work and accomplish tasks by following specific Pharmacy Department and general KTHFS policy and SOPs to accomplish Pharmacy Department priorities.
Must be able to communicate effectively both orally and in writing with patients, caregivers, pharmacy staff, other health care professionals, regulatory representatives and technical support teams.
Possess strong public and interpersonal relationship skills. This is the ability to meet and deal with a variety of individuals exercising cultural awareness, tact, diplomacy, and mature judgment. Must be able to greet and deal with the public in a pleasant, courteous, and professional manner including times of stressful situations.
Must possess the ability to function effectively under time constraints or when multiple tasks are required.
Must attain the skills and abilities required to operate prescription pharmacy computer systems and electronic health record systems.
Knowledge and skill in the general use of computers and various software programs.
Must be able to demonstrate good judgment in the ability to organize and direct workflow, manage ancillary staff and ensure prescription delivery is timely and accurate.
Must demonstrate knowledge and support for continuous quality improvement programs in the department and throughout the organization.
Professional clinical knowledge and ability do drug utilization review for individual patients and at the population level.
Ability to manage inventory, including the maintenance of the controlled substance records; demonstrate working knowledge of inventory control principles to establish/ensure appropriate cost-effective asset management.
Ability to perform all aspects of operating a pharmacy to fill prescriptions through final dispensing to patients in a timely and accurate manner.
Ability to gain knowledge or experience with Native American culture, preferably Klamath Tribal culture, along with disease that disproportionately affect Native Americans.
Ability to maintain strict confidentiality of medical records and adhere to the standards for health record-keeping, HIPAA and Privacy Act requirements. Must maintain strict confidentiality of patient privacy and program information.
QUALIFICATIONS, EXPERIENCE, EDUCATION
Minimum Qualifications: Failure to comply with minimum position requirements may result in termination of employment.
•REQUIRED to possess a current License as Registered Pharmacist (out of state applicants; Oregon Licensure required within 90 days of hire. Must submit copy of Licensure with application.)
•REQUIRED to have PGY-1 Residency training and/or 2 years of applicable experience.
•REQUIRED to be free from exclusion from providing Federal health care benefits including Medicare and Medicaid as per the Federal OIG and GSA exclusion lists.
•REQUIRED to work a flexible schedule with possible evenings.
•REQUIRED to be able to stand and walk around a busy pharmacy at least 80% of the time.

•REQUIRED to acquire and maintain Basic Life Saving (BLS), Advancer Cardiac Life Support (ACLS), and Pediatric Advance Life Support (PALS) certification within 180 days of hire; strongly recommend certification within 90 days of hire, probationary period will be extended for a maximum of 180 days pending certification.
•REQUIRED to complete in a timely manner standard annual required training for health care workers or any training required by KTHFS such as Blood Borne Pathogen, HIPAA Privacy and Security, and others as deemed necessary.
REQUIRED to submit to annual TB skin testing and adhere to KTHFS staff immunization policy in accordance with the Centers for Disease Control immunization recommendations for healthcare workers.
•REQUIRED to submit to a background and character investigation, as per Tribal policy. Following hire must immediately report to Human Resource any citation, arrest, conviction for a misdemeanor or felony crime.
•REQUIRED to submit to pre-employment alcohol and drug testing and occasional random testing.
•REQUIRED to accept the responsibility of a Mandatory Reporter in accordance with the Klamath Tribes Juvenile Ordinance Title 2, Chapter 15.64 and General Council Resolution #2005 003, all Tribal staff are considered mandatory reporters.
Preferred Qualifications:
•Positive working experience with Native Americans in a related field will be given preference.
•Boar Certification.
•Demonstrated working knowledge of the current pharmacy and electronic health systems along with filling technology used at KTHFS.
•Residency training in patient centered outpatient managed care setting providing direct patient care.
•Knowledge of or involvement with the Indian Health Service P&T committee, VA purchasing programs, 340B purchasing programs.
•Experience with pharmacist billing activities, including prescription, medication therapy management (MTM) and collaborative practice agreement billing.
•Collaborative practice agreement development and/or practice under such agreement.
Indian Preference:
Indian and Tribal Preference will apply, as per policy. (Must submit tribal documentation with application to qualify for Indian Preference).
