The Case Manager provides assistance to homeless and formerly homeless Veterans to access entitlements that lead to safe and affordable permanent housing in the community.

• Enters intakes, discharges, and case notes into the HMIS system within 48 hours, or in the case of a holiday by the next business day after a holiday.
• Establishes Housing Services Plan, which includes a thorough assessment of all underlying issues that have led to instability in prior housing situations.
• Assists Veterans in their transition to housing by identifying and providing resources needed for a Veteran specific transition.
• Serves as a liaison between referral/intake sources and community service providers, attending meetings on a biweekly basis.
• Meets with client a minimum of once per month, or as directed by the grant contract.
• Inspects selected units for Housing Quality Standards utilizing up to date HUD criteria.
• Conducts monthly unit inspection for clients and addresses any maintenance and housekeeping concerns with tenant, and reporting maintenance concerns to property management.
• Develops with the Veteran, and monitors, a written housing stability plan that records the Veteran’s shortterm and longterm goals as well as plans of action for physical and behavioral health, substance use treatment, legal, and financial/budgeting matters.
• Supports the Veteran in advocating with landlord and service providers as needed to fulfill the condition of tenancy and to remain housed.
• Provides ongoing case management services and maintains accurate, confidential records in HMIS.
• Assesses the Veteran in their living environment and identifies and resolves barriers to success as they arise.
• Maintains a working knowledge of available Veteran and community resources and the ability to judge when a referral to those resources is appropriate.
• Assists in the preparation of monthly and quarterly reports.
• Participates in regular staff and team meetings.
• Offers recreational and social activities and programming to Veteran ESSHI tenants.
• Is available to work evening hours as may be required to accomplish job responsibilities.
• Performs other duties as directed by management.
POSITION QUALIFICATIONS
• Minimum 2year degree in social work, public administration or other human service field.
• Related work experience in public or private programs providing human services support which includes program planning, administration, public relations and case management.
• Experience working with atrisk populations (homeless, domestic violence, veteran populations, chemical dependency, or mental health). Experience working with veteran population preferred.
• Demonstrated proficiency in computer and written and oral skills.
• Driver’s license and car required with appropriate insurance
SKILLS & ABILITIES

Education: 2-year degree in social work or human services.

Experience: 2 years of full-service case management experience at a non-profit organization or New York State Agency. Experience working with at-risk populations (homeless, domestic violence, veteran populations, chemical dependency, or mental health). Experience working with veteran population preferred.

Computer Skills: Microsoft Word, Excel, Office 365.

Other Requirements: U.S. Military Veteran preferred. Sensitivity to the needs of Veterans and the minority community is required.
