In this role, you will evaluate residents, develop individualized treatment plans, and provide therapeutic interventions designed to maximize independence, safety, and quality of life.Key ResponsibilitiesEvaluate residents and develop individualized occupational therapy treatment plansProvide skilled occupational therapy services in accordance with physician orders and established goalsMonitor resident progress and modify treatment plans as appropriateCommunicate effectively with physicians, nursing staff, residents, families, and interdisciplinary team membersMaintain accurate and timely documentation in accordance with regulatory and payer requirementsParticipate in care planning meetings, rehabilitation conferences, and discharge planning activitiesEducate residents, families, and caregivers regarding treatment goals and home programsConduct in-service education and training for facility staff as neededEnsure compliance with all state practice acts, facility policies, and regulatory requirementsPromote resident safety, independence, and quality of lifeQualificationsEducation & ExperienceBachelor's degree in Occupational Therapy requiredMaster's or Doctorate degree in Occupational Therapy preferredExperience in skilled nursing, rehabilitation, or long-term care preferredExperience with PointClickCare (PCC) and Casamba preferredStrong communication, clinical, and documentation skillsLicenses & CertificationsCurrent Occupational Therapist license in South Carolina in good standingMust maintain all required continuing education requirementsMust remain in good standing with the applicable State BoardWe are an equal opportunity employer.
