Job Description

We are seeking an LPN for our long-term care and rehabilitation healthcare facility. You will provide direct nursing care to residents while supervising the daily activities of nursing assistants.. You will work within a small multidisciplinary team and report directly to the Nurse Manager through the Charge Nurse. This role involves a range of duties including patient assessment, medication administration, documentation, and family communication.

Part-time Weekday 6 pm-12 am

Responsibilities

Assess patient medical history and record vital signs accurately
Administer prescribed medications, injections, and wound care
Monitor patient conditions, including fluid intake/output, and maintain detailed charts
Collect lab samples as assigned
Provide emotional and psychological support to patients
Communicate with patients’ families to provide updates and guidance
Supervise and support nursing assistants in daily care tasks
Ensure compliance with health, safety, and sanitation standards

Required Qualifications

Licensed Practical Nurse (LPN)
Current Tennessee nursing license (LPN )
Current CPR Certification (BLS Provider)

Preferred Qualifications

1+ years of nursing experience in a long-term nursing center
Strong skills in patient assessment, medication administration, and charting
Effective communication and interpersonal abilities
Team collaboration and time management skills
