The position requires a total of 20 hours per month, with one 8-hour day required on-site monthly and the remaining 12 hours completed remotely. Responsibilities include conducting monthly in-person patient visits for comprehensive medication reviews and treatment plan assessments. The clinician will collaborate with the interdisciplinary care team to ensure continuity of care and optimal patient outcomes.
