Description

• Conduct assessments of patients in both inpatient and outpatient environments and create customized treatment plans

• Provide education to patients and their families about therapeutic objectives, discharge processes, and home exercise protocols

• Engage actively in care team meetings and interdisciplinary discussions to promote seamless care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Valid state license as a Physical Therapist
• CPR certification is preferred
