We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our evening shift team at our state-of-the-art healthcare facility in Mount Airy, MD . As an LPN with Mount Airy Nursing and Rehab, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

About the Role:

• Provide skilled nursing care to our patients, including administering medications, performing vital signs, and maintaining accurate patient records
• Collaborate with the healthcare team to develop and implement individualized care plans
• Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
• Demonstrate a commitment to safety, dignity, and respect for all patients, families, and colleagues

Responsibilities:

• Provide routine nursing care, including vital sign monitoring, medication administration, and wound care
• Assist with ADLs, mobility, and ambulation as needed
• Monitor and report changes in patient condition to the healthcare team
• Participate in various committees and projects to improve patient care and facility operations
• Maintain accurate and up-to-date patient records
