- Providing psychosocial assessments
- Providing benefits counseling.
- Development of appropriate disposition planning for each patient. Identifying and providing patients with appropriate mental health/social service referrals as needed.
- Participating as part of a multidisciplinary team including both mental health and medical providers toward coordination of care.
- Working closely with related social and community agencies.
- Providing professional and appropriate interventions in crisis or emergency situations.
- Completing appropriate documentation in the electronic medical record.
- Workload - 25-30 (estimated) assessments weekly - to include new (initial) and current (monitoring) patients, MH/social service/External Provider referrals (up to 10 weekly) - estimation
