Key responsibilities include: Client Assessment and Diagnosis: Conduct comprehensive assessments, identify psychosocial needs, and provide appropriate diagnoses using evidence-based methods. Resource Connection: Develop individualized service plans, linking clients to housing, employment, mental health services, substance use treatment, and other community resources. Counseling and Support: Provide therapeutic support to address trauma, life challenges, and behavioral issues, fostering resilience and personal growth. Life Skills Training: Facilitate workshops on budgeting, job readiness, communication, and other critical skills to help clients achieve independence. Advocacy: Serve as an advocate for clients, ensuring access to public benefits, healthcare, and legal aid, while addressing systemic barriers. Case Management: Monitor progress through regular follow-ups, adjust service plans as needed, and document all interactions and outcomes.
