Care Management Support Coordinator II

Location: San Antonio, Corpus Christi, Dallas, Austin, Houston & McAllen, TX

Pay Rate: Starting at $20.00/hour

Schedule: Monday-Friday | 8:00 AM-5:00 PM

Dean's Professional Services is seeking a dependable, organized, and self-motivated Care Management Support Coordinator II to support a leading healthcare organization. This is an excellent opportunity for an administrative professional who is comfortable working with data, managing multiple priorities, and supporting care coordination services behind the scenes.

Responsibilities

• Support administrative care management activities, including service coordination, scheduling, and documentation.

   

• Perform data entry and accurately maintain member records.

   

• Post and upload required forms to the State and download forms for attachment to member files.

   

• Route documentation and tasks to Service Coordination or the appropriate department for review.

   

• Send emails and tasks requesting corrections to forms when necessary.

   

• Correspond with State representatives by email or phone as needed.

   

• Use reports to track and monitor members requesting services.

   

• Review information, identify key details, and provide clear recommendations to help move cases forward.

   

• Support collaboration between Managed Care Organizations (MCOs), members, service vendors, the State, and TMHP.

   

• Ensure information is processed and communicated accurately and efficiently.

   

• Meet established monthly production and quality metrics .

   

• Work independently while maintaining accuracy and productivity in a fast-paced environment.

   

• Provide backfill support for team members as needed.

   

Qualifications

• High School Diploma or GED required.

   

• 1-2 years of related administrative experience required.

   

• Strong data entry and organizational skills.

   

• Experience with Microsoft Outlook and Excel preferred.

   

• Comfortable using Excel for reporting, VLOOKUPs, formulas, and other basic functions; training may be provided.

   

• Previous healthcare or medical administrative experience is helpful but not required .

   

• Previous remote work experience preferred.

   

• Strong attention to detail and ability to maintain accuracy when handling large amounts of information.

   

• Self-motivated with a strong work ethic.

   

• Ability to work independently with minimal supervision.

   

• Strong written and verbal communication skills.

   

• Ability to meet established productivity and quality standards.
