In this full-time role, you'll provide essential social work services to elderly participants, maintaining a caseload and collaborating closely with the Interdisciplinary Team. Your responsibilities include conducting psychosocial assessments, care planning, and facilitating community connections while ensuring participants' well-being. A Master’s degree in Social Work and at least 2 years of relevant experience are required.
