BLS certification from AHA or ARC may be required. Home health and HCHB experience required. Experience with home health documentation: SOCs, ROCs, recertifications/reassessments, and plan of care development. Experience supervising PTAs. Preferred: orthopedics and neuro experience. Assignment Details Facility Type: Home Health. Shift Name: Days. Shift Details: Monday–Friday, 08:00–16:00. Weekly Hours: 40. Productivity Expectation: 30 points. Coverage Areas: Hawkins, Mineola, Lindale (local travel between patient visits required). Mileage reimbursed between visits from first to last patient. Primary responsibilities: patient assessment, complete plan of care for home health, SOCs, ROCs, recerts/reassessments, supervise PTA. Dress code: scrubs in black, gray, or purple.
