Responsibilities
- Conduct comprehensive OT evaluations for new admissions and ongoing residents to identify functional limitations and therapy needs.
- Develop, implement, and regularly adjust individualized treatment plans aimed at improving activities of daily living, mobility, balance, and safety with transfers.
- Deliver hands-on therapy sessions, document progress, and educate residents and families; train caregivers on adaptive equipment and home modifications.
- Collaborate with an interdisciplinary team (nursing, physical therapy, speech-language pathology, social work) to coordinate care, set measurable goals, and modify plans as residents progress.
- Monitor safety, participate in fall risk mitigation, and contribute to discharge planning and transitions to appropriate levels of care.
- Maintain accurate, timely documentation in the electronic health record and ensure compliance with applicable regulations and facility policies.

Requirements
- Active Occupational Therapist license in Washington state.
- Ability to work effectively in a skilled nursing facility environment.
- Strong written and verbal communication skills; solid documentation practices.
- Availability to start on 12/07/2026 and commit to a 13-week travel assignment.
- Experience in geriatric rehabilitation or SNF settings is preferred.
