Job Description

Homeside Rehab is seeking compassionate and motivated Physical Therapist (PT) and Physical Therapist Assistants (PTAs) to provide one-on-one home-based therapy services throughout Sullivan County, NY , including Swan Lake, Fallsburg, Woodridge, and Monticello .

At Homeside Rehab, we provide high-quality rehabilitation services in the comfort of our patients' homes. Job Responsibilities

Provide skilled PT treatment services under the direction and supervision of a licensed Physical Therapist or Occupational Therapist
Carry out established plans of care and individualized treatment programs
Provide therapeutic exercises, functional activities, mobility training, and other interventions within your professional scope of practice
Monitor patient progress and communicate changes or concerns to the supervising therapist
Maintain accurate and timely clinical documentation in accordance with company and regulatory requirements
Communicate professionally with patients, caregivers, supervising therapists, and office staff
Provide one-on-one therapy services in patients' homes throughout assigned service areas

Job Qualifications

Active New York State PT OR PTA license and regestration
Basic computer proficiency for electronic documentation
Strong interpersonal and communication skills
Ability to work independently while following the established plan of care and clinical supervision requirements
Valid driver's license and reliable transportation for home visits
Home health, outpatient, rehabilitation, or skilled nursing experience preferred
Minimum of 1 year of clinical experience preferred

Job Details

Job Type: Full-Time, Part-Time, Per Diem
Location: Sullivan County, NY – Swan Lake, Fallsburg, Woodridge, Monticello, and surrounding areas
Pay: $45–$70 per hour , based on discipline, experience, and case assignment
Schedule: Flexible – choose your availability
Shifts: Day and evening availability, Monday through Friday, with optional weekend availability
