Job Description

Are you a Qualified Intellectual Disability Professional (Case Manager) who is flexible in your approach to supporting individuals with disabilities? Look no further than Active Visions in Chicago, IL!

As a QIDP at Active Visions, you will be responsible for providing specific and person-centered support to individuals with intellectual disabilities. Your flexible approach will allow you to adapt to the unique needs and preferences of each individual you work with, ensuring that they receive the highest quality of care and support.

Pliability is key in this role, as you will be required to work with individuals in a variety of settings, including their homes, day programs, and community outings. Your ability to be coachable and adaptable will be crucial in helping individuals achieve their goals and live fulfilling lives.

In addition to being flexible in your approach, you must also be able to collaborate effectively with a team of professionals, including therapists, doctors, and other support staff. Your approach in working with others will ensure that individuals receive comprehensive and person-centered care that meets all of their needs.

Flexibility is not just a requirement in this role - it is a mindset. As a QIDP at Active Visions, you will be expected to think outside the box, problem-solve creatively, and constantly adapt to the changing needs of the individuals you support.
