Person Centered Services is seeking passionate Care Coordinators to work within a health home model, coordinating health, behavioral healthcare, community supports, and other services for individuals with intellectual and developmental disabilities. You will advocate for the person’s needs using a strengths-based, person-centered approach. Responsibilities include developing Life Plans with the individual and their support circle, maintaining a respectful work environment aligned with DEI #J-18808-Ljbffr
