About the Role

The Community Health Worker / Enrollment Specialist is often the first person a participant meets from Yumlish.

In this role, you’ll connect with individuals by phone, help determine whether they’re eligible for our diabetes prevention program, answer questions, and guide them through enrollment. You’ll help people understand the value of the program, address concerns, and encourage them to take the first step toward improving their health.

We’re looking for someone who enjoys talking with people, can build trust quickly, and genuinely cares about helping others. Many of the people we serve are navigating challenges related to food, finances, caregiving, language barriers, transportation, or the healthcare system itself. The ability to listen with empathy and connect with people from different backgrounds is essential.

As Yumlish continues to grow, there may be opportunities to support participants beyond enrollment through community outreach, resource navigation, health education, and other activities that help people stay engaged in their health journey.

Responsibilities

Outreach & Enrollment

* Conduct outbound call campaigns to individuals who may be eligible for the program

* Build rapport and establish trust with prospective participants

* Screen participants using eligibility criteria and structured questionnaires

* Explain program benefits, expectations, and next steps

* Guide eligible participants through the enrollment process

* Follow up with prospective participants who have not yet enrolled

Participant Engagement

* Support newly enrolled participants through their first program session

* Re-engage participants who miss early sessions

* Answer questions and address concerns related to participation

* Help participants overcome barriers to getting started

Documentation & Reporting

* Document participant interactions in CRM systems

* Maintain accurate and timely records

* Track outreach activity, enrollment outcomes, and participant follow-up

* Share enrollment updates and feedback with the Yumlish team

Community Support

* Listen for barriers that may affect participation and communicate those insights to the team

* Support community outreach and participant engagement efforts as needed

* Participate in ongoing training related to diabetes prevention, communication, and community health

What We’re Looking For

* Community Health Worker certification or training

* Must reside in California

* Strong verbal communication skills and comfort speaking with people by phone

* Ability to build trust and motivate people to take action

* Strong organizational skills and attention to detail

* Comfortable using CRM systems and technology tools

* Ability to work independently in a remote environment

* Experience in outreach, enrollment, customer service, patient engagement, community health, or related roles

Preferred Qualifications

* Community Health Worker (CHW) certification or training

* Experience working with Medicaid populations

* Experience in healthcare, public health, community-based organizations, social services, patient advocacy, or care navigation

* Bilingual English and Spanish

* Experience conducting outreach, enrollment, recruitment, or engagement campaigns

Compensation

* Starting rate: $18/hour
