Physical Therapist (PT) – Full-Time & PRN
Aventura at Carriage Inn – Dayton, OH
Aventura at Carriage Inn is growing our in-house rehabilitation team!
We are seeking a motivated and compassionate Physical Therapist (PT) to join our team in Dayton, Ohio . Our primary need is for a Full-Time PT , with PRN opportunities also available .

Position ResponsibilitiesEvaluate residents and develop individualized physical therapy treatment plans
Provide skilled PT services to our SNF/LTC population
Help residents improve strength, mobility, balance, endurance, and overall functional independence
Establish measurable goals and monitor resident progress
Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team
Complete accurate and timely clinical documentation
Utilize Net Health and PointClickCare (PCC)
Participate in discharge planning and resident/family education
Maintain compliance with facility, state, and federal guidelines
Graduate of an accredited Physical Therapy program
Current or eligible Ohio Physical Therapist license
Strong communication and interpersonal skills
Passion for providing quality, resident-centered care
SNF/LTC experience is a plus, but not required
