This position offers the convenience of a remote work setting, enabling you to conduct assessments, therapy sessions, and consult with colleagues from the comfort of your home. Although primarily remote, our LCSWs are integral members of an interdisciplinary team and will engage with healthcare professionals and community services to develop comprehensive care plans for our patients. You will have access to advanced telehealth tools and electronic medical record systems to streamline your work and maintain accurate documentation.

Key Responsibilities\n

• Conduct thorough biopsychosocial assessments of patients to identify mental health, substance use, and medical needs, utilizing culturally sensitive approaches.

• Provide individual and group counseling services, developing treatment plans that address the unique challenges faced by each patient and their families.

• Perform risk assessments, documenting findings and collaborating with healthcare providers to escalate care as necessary, ensuring timely intervention.

• Serve as a resource for patients and their families by offering coaching, support, and referrals to community resources and medication assistance programs.

• Collaborate with the Quality Improvement team to design effective data collection processes that enhance the quality and outcomes of service delivery.

Patient Population / Client Focus\n

You will primarily work with patients facing terminal illness and their families, providing support to help navigate the emotional and psychological complexities associated with end-of-life care. This role requires cultivating a deep understanding of grief, loss, and the importance of empathy as you assist patients in finding peace and dignity in their remaining time.

Qualifications\n

• A valid state license as a Licensed Clinical Social Worker (LCSW) is required.

• A Master’s degree in Social Work with at least three years of post-licensure experience in mental health or related fields is essential.

• Strong foundation in counseling techniques, crisis intervention strategies, and the ability to exercise independent judgment in providing direct services.

• Demonstrated experience in group facilitation and mastery of planning processes that promote effective therapy practices.
