Team Coordinator - Stillwater Clinic

Job Overview Fairview is seeking a dedicated Clinic Team Coordinator to join our hardworking team in Stillwater, Minnesota. Clinic hours are Monday through Thursday from 7:00 a.m. to 6:00 p.m. and Friday from 7:00 a.m. to 5:00 p.m. This position is approved for 80 hours every 2 weeks. As an integral member of the primary care team, the team coordinator contributes to overall quality of care and patient experience by performing high-level functions to support the needs of the healthcare team and patients/support network. The TC partners with all members of the team to ensure patient needs are optimally addressed. They deliver direct support to patients on the coordination of medical information requests and updates, as well as referrals and appointment scheduling. The TC uses a variety of communication tools and systems in their work to support patients and their care team. They also maintain complete, timely and accurate documentation for each patient and continually look for opportunities to improve processes and workflow. Performs telephone & MyChart message support & follow-up communication in a timely and professional fashion; uses communication systems for both routine & emergency transactions Schedules, reschedules & maintains patient appointments & supports clinic leadership in schedule management Partners collaboratively with the patient’s care team to address patient needs Find opportunities for improvement in workflow & participate in clinic and system meetings Required Qualifications Successful completion of formal medical training program such as MA, HUC, NA or Medical Secretary, or min. of 1 year comparable experience in a healthcare setting Preferred Qualifications 2 or more years’ experience working in a medical setting Benefit Overview Fairview offers a generous benefit package including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more!
