Licensed Clinical Social Worker (LCSW) - Riverhead, New York
We are seeking a highly skilled and compassionate Licensed Clinical Social Worker (LCSW) to join our team at CHE Behavioral, a multispecialty group of licensed professionals dedicated to increasing access to mental health services. As an LCSW with CHE Behavioral, you will have the opportunity to work with a diverse patient population, providing high-quality clinical services to improve the quality of life for older adults in long-term care and rehabilitation centers.
Responsibilities:
- Provide licensed clinical social work services to patients in a long-term care and rehabilitation setting, including assessment, diagnosis, treatment, and counseling
- Develop and implement individualized treatment plans to meet the unique needs of each patient
- Collaborate with interdisciplinary teams, including psychiatrists, psychologists, and other healthcare professionals, to ensure comprehensive care
- Conduct individual and group therapy sessions, as well as family therapy as needed
- Monitor patient progress and adjust treatment plans accordingly
- Maintain accurate and thorough records of patient interactions and treatment plans
- Stay current with industry developments and best practices in mental health care
