Required Skills:

• Valid RN license in Texas (or compact license)
• Minimum of 5 years of clinical practice with relevant patient populations
• Strong knowledge of discharge planning, resource management, and care coordination
• Proficiency in word processing within a Windows environment
• Experience with MIDAS, Meditech, and familiarity with Milliman (MCG) Guidelines and Interqual criteria preferred
• Excellent communication skills and ability to analyze and generate reports
• Bilingual in English/Spanish (preferred)
• CPR certification (preferred)

Nice to Have Skills:

• Case Management or Discharge Planning certification
• Previous experience in pediatric care or complex patient management
• Familiarity with healthcare systems and documentation software
• Ability to adapt care plans for cultural and age-specific needs

Preferred Education and Experience:

• BSN preferred; associate degree in Nursing required
• Certification in case management highly desirable
• A minimum of five years in clinical practice within relevant healthcare settings

Other Requirements:

• Willingness to work some weekends and after-hours as needed
• Travel as required for the role
