Licensed Practical Nurse (LPN) - Long-Term Care Travel Contract in New Lisbon, WI

Seeking a compassionate Licensed Practical Nurse (LPN) for a 13-week long-term care travel contract in New Lisbon, Wisconsin. Join a supportive LTC healthcare team and provide high-quality resident care in a welcoming central Wisconsin community.

Job Details

Location: New Lisbon, WI
Setting: Long-Term Care
Assignment Length: 13 weeks
Shift: Full-time evening shift, 2:00 PM - 10:00 PM
Weekend Requirement: Every other weekend
Weekly Hours: 36 guaranteed hours per week
Estimated Weekly Pay: $1,500 - $1,580
Start Date: 10/19/2026

Job Requirements

Active Wisconsin LPN license required
Current CPR/BLS certification required
At least 1 year of related nursing experience required
Strong attention to detail and excellent medication administration accuracy
Ability to work collaboratively with CNAs, charge nurses, physicians, and families

Responsibilities

Administer medications to residents in accordance with the 5 Rights of medication administration
Monitor residents for changes in condition and promptly report updates to the charge nurse
Communicate resident updates to physicians and family members as needed
Assist CNAs with call lights and daily resident care support
Report allegations of abuse, neglect, or misappropriation to leadership immediately
Maintain accurate documentation and complete additional duties as assigned
