About the Role The clinical coordinator will support individuals living in supportive housing with complex needs by assessing and respond to clinical issues impacting a resident’s ability to thrive in supportive housing. Responsibilities Provide direct support and/or linkage to referrals for a small caseload of residents who are living in Bridge housing throughout the Bronx and Manhattan. Provide clinical direction and guidance to supportive housing staff. Work in collaboration with internal and external stakeholders for increased coordination of care. Utilize agency electronic health record system to document services and track data. Promote physical and mental health, safety, wellbeing, and recovery with clients. Qualifications Master’s Degree or higher in one of the following areas – Social Work or Mental Health Counseling. Experience working with people in supportive housing with mental health diagnoses is a plus. Required Skills Ability to use agency Electronic Health Record to document services.
