Care Coordinator

Job Definition The Care Coordinator functions as a member of an interdisciplinary team to provide care coordination to adults diagnosed with a severe mental illness, with multiple medical comorbidities and/or co-occurring substance abuse disorders. The Care Coordinator advocates for and supports the client, engages with community agencies, health care providers, and others on the client's behalf to ensure access to services, increase wellness self‑management, and reduce emergency room visits and/or hospitalizations. The Care Coordinator will provide expert support and psychosocial and/or substance abuse interventions, and resources to aid in the client's care while maintaining focus on outcomes and best practices. Educational Requirements Preferred bachelor’s or master’s degree in one of the following fields: Social Work Psychology Education Rehabilitation Occupational Therapy Counseling Community Mental Health Sociology Speech and Hearing Physical or Recreational Therapy Degrees in other related areas may be considered. Experience Required For high school diploma/GED candidates: at least 4 years of related human service and direct service experience. For bachelor’s degree candidates: at least 1 year of related human service and direct service experience. Experience should include providing direct services to individuals with severe mental health disorders, co‑occurring substance abuse disorders, developmental disorders, and/or physical disabilities, linking clients to a broad range of services, and advocating for underserved or disenfranchised populations. Bilingual language skills preferred. Working knowledge of computer software and electronic health record systems. Demonstrated competency in written, verbal, and computational skills for documenting records in accordance with program standards. Experience and comprehensive knowledge of the interdisciplinary planning process and the developmental treatment model. Knowledge of Medicaid, Social Security, and other entitlements preferred. Excellent interpersonal skills required. Knowledge of cultural competence and sensitivity. Ability and willingness to travel regularly, sometimes with clients in agency vehicles, to many locations using reliable and safe transportation. If working in Nassau or Suffolk Counties, a valid Tri‑state driver’s license is required. Duties and Responsibilities Conduct initial and ongoing assessments of assigned clients to document strengths, needs, goals, and resources. Participate in the development, documentation, review, and update of client‑centered comprehensive, interdisciplinary care plans with Program Supervisors and team members. Maintain effective communication with clients, primary care physicians, substance abuse and mental health providers, families, collateral resources, and agency staff on behalf of clients. Maintain records, statistics, and related reports in an organized, timely, and accurate manner per policy and procedure. Coordinate care planning with other service providers to ensure goal‑directed, collaborative care, including care transitions. Participate in team meetings to provide input and feedback on psychosocial conditions and comorbidities. Act as a resource and consultant for team members on psychosocial and substance abuse issues. Provide telephonic and face‑to‑face outreach, engagement, and service planning in the field. Serve as a linkage to community services, including medical, behavioral, residential, entitlement, and other needed services. Monitor overall service delivery to ensure coordination and continuity, advocating with providers as needed. Provide crisis intervention and follow‑up on an as‑needed basis 24/7. Participate in ongoing supervision, training, and education to ensure high‑quality service delivery and professional growth. Facilitate sharing of data with clients to support shared decision‑making and keep clients informed of progress. Complete QI training during onboarding and participate in refresher training as needed. Cultivate an inclusive culture that respects individual strengths, views, and experiences. Adhere to all safety protocols and procedures to ensure a safe working environment. Demonstrate commitment to health and safety awareness. Other duties as assigned.
