Acute Care Case Manager

This is a 13-week contract position.

Day Shift: 5x8-Hour (08:00 - 16:30)

The Acute Care Case Manager will coordinate patient care to ensure efficient discharge planning and appropriate resource utilization. This role requires strong clinical judgment and effective communication skills to work in collaboration with the interdisciplinary team. The successful candidate will provide case management for a patient load of up to 25 patients with an emphasis on quality care outcomes and compliance with regulatory requirements.

Responsibilities:

• Conduct comprehensive patient assessments and care coordination.
• Develop and implement individualized discharge plans.
• Participate in family meetings to discuss treatment plans.
• Ensure compliance with CMS regulations and departmental policies.
• Arrange durable medical equipment (DME), skilled nursing facility (SNF) admissions, and ambulance transport as required.

Required Experience / Certifications / Licensure:

• 2 years of acute care case management/discharge planning experience in a hospital setting.
• California state license (pending license accepted but must be licensed upon start date).
• Experience with Epic charting system is required.
• Must possess knowledge of care coordination and eligibility for benefits.
• Experience in addressing needs of patients with varied backgrounds, including those experiencing homelessness or mental health issues.
