Field-Based Community Health Advocate for Older Adults

Matter Health

Matter Health seeks a Patient Advocate & Liaison to engage older adults in public housing communities across Georgia. Working with an NP, you’ll educate patients, support enrollment, and coordinate follow-up to keep people connected to care.

In this field-based role you’ll build trusted relationships, conduct outreach in-person and via calls, help patients navigate services, and document activities in Salesforce and Athena. A driver’s license is required.
