Schedule: Monday-Friday, 9:00 AM-5:00 PM

About the Role

Our client is seeking an experienced and compassionate Care Management Supervisor to lead and support a team of Health Home Care Managers serving individuals throughout Nassau and Suffolk Counties.

This is an excellent opportunity for an experienced Care Manager or current supervisor who is ready to take the next step into a formal leadership role while remaining connected to the clients and communities being served.

The Care Management Supervisor will provide day-to-day leadership, mentorship, and guidance to Care Managers supporting individuals with Serious Mental Illness (SMI), substance use disorders, frequent hospitalizations, chronic health conditions, homelessness, and housing instability .

This is a hands-on leadership position . While the role includes administrative and supervisory responsibilities, the Supervisor will also remain

active in the field, participating in community visits, assisting with complex cases, supporting high-risk interventions, and collaborating with hospitals, shelters, healthcare providers, and community organizations.

Key Responsibilities

Lead & Mentor a Team

• Supervise and support a team of Health Home Care Managers.
• Provide regular individual and team supervision.
• Coach and mentor staff through complex cases and challenging client situations.
• Conduct case reviews and provide guidance on interventions, care plans, and service strategies.
• Support employee development, training, performance management, and professional growth.
• Assist staff with high-risk situations, safety planning, crisis intervention, and incident reporting.

Provide Field-Based Support

• Participate in community-based client visits throughout Nassau and Suffolk Counties.
• Visit clients in homes, shelters, hospitals, and other community settings.
• Provide hands-on support during high-risk or complex interventions.
• Assist Care Managers with difficult cases and help identify appropriate solutions.
• Support clients following emergency room visits, hospitalizations, or other transitions in care.
• Advocate for clients and help address barriers to healthcare, behavioral health, housing, and social services.

Ensure Quality & Compliance

• Review progress notes, assessments, care plans, and other documentation within the EHR.
• Ensure Care Managers maintain accurate and timely documentation.
• Monitor caseload assignments and service delivery.
• Ensure staff follow Health Home policies, procedures, and regulatory requirements.
• Monitor client outcomes and identify opportunities to improve quality and continuity of care.
• Utilize Outlook, Word, Excel, EHR systems, and other organizational tools.

Build Community Partnerships

• Develop relationships with hospitals, shelters, behavioral health providers, healthcare organizations, and community-based agencies.
• Collaborate with community partners to resolve complex barriers to care.
• Help coordinate timely follow-up after ER visits and hospitalizations.
• Connect clients and staff with appropriate community resources and service providers.

Required Qualifications

• Bachelor's degree required in Social Work, Psychology, Sociology, Education, Nursing, Human Services, Healthcare Management, or a related field.
• Master's degree strongly preferred.
• At least 3 years of experience in case management, care coordination, Health Home services, or a related field.
• Experience working with individuals with Serious Mental Illness (SMI) and/or homelessness or housing instability.
• Experience supporting individuals with complex behavioral health, medical, or social needs.
• Demonstrated leadership, coaching, and staff-support experience.
• Strong crisis management, communication, organization, and problem-solving skills.
• Strong EHR and documentation skills.
• Proficiency with Microsoft Office, including Outlook, Word, and Excel.
• Willingness and ability to travel throughout Nassau and Suffolk Counties .

Preferred Qualifications

• Previous experience with the New York State Health Home Care Management system .
• Prior supervisory or team leadership experience.
• Experience working with shelters, hospitals, behavioral health providers, or community-based organizations.
• Experience supporting individuals with substance use disorders, frequent hospitalizations, chronic conditions, or complex social needs.

Work Environment & Schedule

This is a hybrid position combining office, remote, and community-based work.

Schedule: Monday-Friday, 9:00 AM-5:00 PM

The Supervisor should be comfortable commuting between the Copiague and Nassau County locations as needed. The locations are not expected to be visited on the same day.

The role also requires regular travel throughout Nassau and Suffolk Counties for client visits, staff support, hospital visits, shelter visits, and community-based interventions.
