Communicate and advocate for patient and family goals as part of the interdisciplinary team; coordinate care across transitions and provide developmentally appropriate, culturally sensitive education. Function effectively within nursing and inter‑professional teams; support shared decision‑making and apply practices that minimize risks during handoffs and transitions of care. Integrate best available evidence with clinical expertise and patient/family preferences; participate in quality improvement activities and identify opportunities to revise policies or practice guidelines as appropriate. Ensure safety through adherence to policies and procedures; recognize and report unsafe conditions and participate in routine documentation in the electronic medical record (EMR).
