Field LTSS Service Coordinator | Care Team Leader

Elevance Health is seeking a LTSS Service Coordinator (Case Manager) to support a defined caseload in Wisconsin. This field-based role requires travel to client sites and a mix of in-office training when needed, aligning with our hybrid-work approach. You will conduct assessments, develop Person Centered Support Plans, coordinate with medical directors and an interdisciplinary team, and help members achieve goals while managing PH/BH/LTSS needs.
