The Contractor shall furnish all key personnel to provide services necessary to perform onsite and on-call Interventional Radiology Physician Services to eligible beneficiaries of the Department of Veterans Affairs, VA New York Harbor Health Care System, Brooklyn Campus. The Contractor's physicians care shall cover the range of Interventional Radiology services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by the American College of Radiologists Guidelines. Contractor physicians shall be accredited by ACR for Interventional Radiology. Place of Performance: Services shall be provided on site at the Department of Veterans Affairs, Brooklyn VA Medical Center, located at: New York Harbor Health Care System Brooklyn VA Medical Center 800 Poly Place Brooklyn, NY 11209-7104. There are no alternate campuses of this Medical Center in which contractors will need to travel to/from to perform services. Schedule: 1.0 FTE (2,080 hours a year) onsite services are required Monday through Friday, 08:00 – 16:30. At least four candidates should be credentialed and readily available to provide services. On-call is needed during WHEN hours ("WHEN" hours are defined as Weekend, Holiday, Evening & Night hours) from 16:30- 0800 on weekdays, and full weekend coverage from 16:30 Friday through 08:00 Monday. Call-back hours are estimated. The hourly rate changes from on-call to call-back only upon arrival at the VA facility to perform onsite services. Pricing Instructions: The offeror is instructed to edit the number of sub-line item numbers (SLIN) to correspond with the number of key personnel submitted for the line item number (LIN). Renumber SLINs if adding or removing Key Personnel. To meet the full requirement, the Contractor shall propose a minimum of four key personnel to be credentialed and be available for scheduling to meet the requirements of the contract. Offerors are encouraged to submit more than four candidates with their offer. Please note that one FTE is defined by VA as 2,080 hours a year. The offeror is to utilize the price schedule below. Partial offers may be submitted but must clearly state the amount of FTE being proposed in the same format as the price schedule. Rates shall be entered to reflect hourly rates. All rates are to be broken out as requested and shall be all-inclusive of any travel/lodging rates. Onsite services are provided during regular VA business hours, Monday through Friday 07:00am – 15:30pm, minus holidays. On-call services are provided during "WHEN" Hours.
