Hospice RN Case Manager: Compassionate Care Lead

Allina Health is seeking an experienced Hospice Case Manager to lead compassionate, personalized care for patients and families in a hospice setting. You will assess needs, develop and adjust care plans, and collaborate with a multidisciplinary team to deliver high-quality end-of-life care. The role requires RN licensure in MN or WI, an Associate or Vocational degree, and 2+ years of RN experience; BLS certification is required, with CHPN and CMGT-BC preferred. #J-18808-Ljbffr Allina Health
