CCAP is seeking an HEZ Outreach and Engagement Specialist to provide outreach for Cranston residents and patients in the Health Equity Zone panel. You will build and operate social media outlets to reach a broad community, educate residents on HEZ initiatives, and collaborate with partner organizations to connect people with available services.
