Case Manager

PRINCIPAL DUTIES AND RESPONSIBILITIES:
  • Conduct needs assessments centered on the daily living needs of patients and barriers to daily living functions, helping the patient identify problems that he/she may not initially express or identify as difficulties.
  • Evaluates appropriate health care services and treatment plans in conjunction with providers.
  • Ensures that case management program objectives are met by evaluating the effectiveness of alternative care services and that are cost effective, quality care is maintained.
  • Performs case management for specialty programs in a telephonic customer/ in-person - facing environment which involves utilizing a coaching approach that provides support and resources to the patients.
  • Perform HEDIS/QARR related tasks including chart audits, monitoring, and tracking HEDIS/QARR result.
  • Develop SMART goals or Action plan with patients to empower them to lead and maintain a healthy lifestyle.
  • Help patients navigate access to services - providing encouragement, information and teaching the skills necessary for a patient to get to and use a service.
  • Provide outreach to patients for scheduling appointments, screenings, reminders, and missed opportunities.
  • Provide care coordination among patients and outside referral entities.
  • Ensure patient-centered communication linkages among the center's direct service providers.
  • Accesses services and situations for both immediate and long-term needs of clients, including identification (ID) information, transitional and permanent housing, income maintenance and insurance benefits.
  • Participate in PRAPARE, Care Coordination, CQI and Population Health Management (PHM) team meetings and committee activities.
  • Build awareness of care coordination services among the health care team by building professional relationships with team members, providing information about connecting service and maintaining communication to identify patients who are "at risk" for barriers to care.
  • Ability to work in a demanding, fast-paced environment with constant public contact, frequent interruptions, and occasional crisis situations.
  • Ability to understand and respond effectively and with sensitivity to special population groups, including those defined by race, ethnicity, language, age, gender, sexual orientation, economic standing, & others.
Requirements

KNOWLEDGE, EDUCATION, SKILLS AND ABILITIES REQUIRED:
  • Education - An Associates (A.A.) degree with 3 years of experience, or Bachelor's degree with 1+ years of experience in a clinical, medical or mental health setting.
  • Experience as a certified Medical Assistant, preferred.
  • Minimum (1) years of experience working with Behavioral Health and/or Substance Use disorder patients is preferred.
  • Proficiency in use of EMR, experience in medical chart review, including MS Excel, MS Word, PowerPoint and internet navigational skills; and highly developed interpersonal and communication skills.
  • Bilingual Preferred. Proficiency in English and Spanish or Chinese, both written and spoken language.
  • Experience working in medical settings and interacting collaboratively with medical teams. Possess a working knowledge of HIPAA regulations; FQHC experience preferred.
  • Excellent communication and organizational skills with professional demeanor.
