Description

American Traveler is hiring an LPN/LVN for a Home Health & Hospice setting, requiring 2+ years of Home Health experience and hospice experience.

Details

• Home Health and Hospice work setting focused on routine visits
• Patient ratio of 1:30
• Epic charting system experience required
• Day shift schedule, 5x8 hours, 8:00 AM-4:30 PM
• Possible evening/night on-call responsibilities
• May be required to float within the health system as operationally necessary

Requirements

• Washington LPN/LVN license required if applicable
• Minimum 2 years of experience in Home Health required
• Hospice experience required
• BLS certification required
• Epic charting experience required
• Two professional references from the past 2 years required, at least one from a supervisor
• Proof of Washington state license application required if license not yet in hand
• Travel healthcare experience required
• Candidates with more than one DUI are not eligible for consideration

Additional Information

• Provide routine home health visits including hospice care
• Previous PeaceHealth employees must be separated from the health system for at least 1 year before returning as a traveler
• First-time travelers will not be considered for this position
• Local candidates within 60 miles of the facility are not accepted
• RTO must be disclosed and approved prior to contract signing, with no additional RTO approved afterward
• Start date may be delayed up to 8 weeks if Washington license is still in process
• Proof of full vaccination status must be disclosed
