Job Description:

Make an impact as a Case Manager by leading patient-centered care coordination from admission through discharge. In this role, you will manage a dynamic caseload, assess patient needs, and collaborate with physicians, care teams, and families to develop and execute effective, cost-conscious care plans. You'll play a key role in streamlining care delivery, removing barriers, and ensuring smooth transitions across the continuum of care.

32 Weekly Hours, Full Time, Dayshift (will work rotating weekends)

Requirements:

Education:

• Bachelor's degree required (Equivalent combination of education and relevant clinical experience may be considered in place of a degree)

Experience:

• Minimum of 3 years of job-related experience required (Experience in an acute care setting required, preferably within the area of practice)

Licensure/Certification:

• Active RN license with the Kentucky Board of Nursing or a valid compact RN license
• Case Management Certification (ANCC, CCM, or ACM-RN) - Required within 2 years of hire.
