Physical Therapist

The Physical Therapist is responsible for assessing and treating patients to restore physical function and improve mobility within a skilled nursing facility setting. This role involves designing and implementing individualized treatment plans, educating patients and families, collaborating with an interdisciplinary healthcare team, and maintaining accurate documentation to support patient recovery and enhance quality of life. Responsibilities:

• Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.
• Provide physical therapy interventions to improve strength, mobility, and functional abilities.
• Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.
• Collaborate with occupational therapists, nurses, physicians, and other healthcare team members to coordinate care and optimize patient outcomes.
• Monitor patient progress and adjust treatment plans as necessary to ensure effective outcomes.
• Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in compliance with facility policies.
• Ensure care plans meet regulatory requirements and align with best practices in physical therapy.

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Valid Physical Therapy license in the state of practice.
• Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work independently and collaboratively within a healthcare team.
