About This Role The Field Nurse in the Home Care Division at NYU Langone Health provides safe, quality nursing care using evidence-based practices. Highlights Pay: $116,515.08 - $141,265.08 Annually Location: Bethpage, NY Facility: NYU Langone Hospital—Long Island Department/Unit: Home Care Division Schedule: Full-Time, Day shift Sign-On Bonus: $5,000 What You'll Do Completes a comprehensive nursing assessment including physical, functional, psychosocial, and other factors. Derives diagnoses and needs from assessment data and collaborates to develop an individualized plan of care. Coordinates care by interacting with other healthcare team members to optimize patient outcomes. Delivers direct nursing care, ongoing assessment, teaching, and training to patients and caregivers. Identifies deficits and barriers to effective patient education and employs strategies to optimize engagement. Maintains accurate and timely documentation reflecting assessments, diagnoses, and plans of care. Assists Agency leadership in achieving corporate goals and improving patient care outcomes. Participates in quality assessment and performance improvement initiatives.
