Alaris Health - Axia Care Center is a long-term care and subacute Skilled Nursing Facility (SNF) committed to delivering high-quality, patient-centered care. We are seeking an experienced Social Worker to support our residents and families while collaborating closely with the interdisciplinary care team. Key Responsibilities Conduct psychosocial assessments and develop individualized care plans Support residents and families with adjustment, discharge planning, and long-term care transitions Coordinate with nursing, admissions, therapy, and case management teams Ensure compliance with state and federal regulations Participate in care plan meetings, family meetings, and discharge planning conferences Maintain accurate and timely documentation in the electronic medical record Serve as an advocate for residents’ rights, dignity, and overall well-being Qualifications Bachelor’s degree in Social Work (BSW) required; LSW or CSW preferred Active social work license or certification per state requirements Must have prior experience working in a Skilled Nursing Facility (SNF) Strong communication, organizational, and problem-solving skills Ability to function effectively in a fast-paced, team-oriented environment What We Offer Competitive salary Supportive leadership and a collaborative work culture Opportunity to make a meaningful impact on residents’ lives Professional growth and advancement opportunities Axia Care Center is an equal opportunity employer. #J-18808-Ljbffr
