Key Responsibilities
- Evaluate patients and develop individualized physical therapy treatment plans within a skilled nursing facility setting
- Administer therapeutic interventions to improve patient mobility, strength, and functional independence
- Monitor and document patient progress, adjusting treatment plans as clinically appropriate
- Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
- Educate patients and family members on home exercise programs and safe functional techniques
- Active Physical Therapist (PT) license in the state of Minnesota or eligibility to obtain licensure
- Graduate of an accredited Physical Therapy program
- Experience working in a skilled nursing facility (SNF) or long-term care setting
- Strong clinical documentation skills and knowledge of regulatory compliance standards
- Prior contract or travel therapy experience
- Familiarity with MDS documentation and PDPM reimbursement models
