Job Summary

Assist in maintaining a physical, social and psychological environment that is conducive to the best interest and welfare of residents. Administer or assist with self-administration of medication and treatments as prescribed by the health care provider and observe/report responses to existing care plans based on state regulations and completion of required training/certification. Maintain cleanliness and audit medication supply room or medication carts as appropriate and provide accurate accounting of all medications.

Accountabilities

1. General
   • a) Model the Culture Change Principles of the Eden Alternative.
   • b) Understand and adhere to Resident Confidentiality and HIPAA regulations.
   • c) Attend in-service training, educational classes, and meetings as required.
   • d) Read and comprehend instructions, correspondence and memos.
   • e) Communicate and function in an interdisciplinary team.
   • f) Maintain a thorough working knowledge of Community Electronic Health Record software.
   • g) Maintain a thorough working knowledge of Community policies and procedures.
   • h) Perform basic mathematic procedures.
   • i) Make independent decisions when circumstances warrant such action.
   • j) Maintain the highest degree of personal and professional integrity.
   • k) Know and honor Resident Rights.
   • l) Communicate with residents in a respectful manner.
   • m) Respond to resident family/visitor requests in a timely and appropriate manner.
   • n) Provide effective and courteous service to all residents, guests and co-workers.
2. Medication Administration
   • a) Ensure the medication room door or medication cart is locked at all times.
   • b) Maintain neat, clean and stocked medication cart.
   • c) Order, store and administer medications according to Community policies and procedures and current physician orders.
   • d) Complete narcotic count on each shift, immediately reporting any concerns to the Director of Nursing.
   • e) Read the daily notes for information and changes at the beginning of each shift.
   • f) Disposal of expired or discontinued medications in a timely manner according to Community policies and procedures.
   • g) Obtain & record vital signs as directed on the Medication Administration Record [MAR] or vital sign log in resident's electronic health record.
   • h) Adhere to Universal Precautions and Infection Control procedures.
   • i) Maintain medication and specimen refrigerator temperature logs.
3. Observation, Documentation and Reporting
   • a) Regularly review resident care plans and medication orders to be aware of additions or changes to required support and assistance.
   • b) Complete Incident Reports per Community Policy and Procedure.
   • c) Complete Short Term Observation charting in resident’s Electronic Health Record [EHR] when/if applicable.
   • d) Document support and assistance provided in the resident’s Electronic Health Record [EHR] when/if applicable.
   • e) Document and report resident and/or family requests for additional or changes in support/assistance in the resident’s Electronic Health Record [EHR] when/if applicable.
   • f) Document and report observations including unusual or significant changes in resident physical, mental or behavioral conditions in the resident’s Electronic Health Record [EHR] when/if applicable.
4. Resident Care
   • a) Recognize and respond to the needs of Elders.
   • b) Promote autonomy of Elders in decision making.
   • c) Maintain confidentiality and privacy of Elder care, procedures, and documentation.
   • d) Communicate with co-workers at all levels to meet the needs of Elders.
   • e) Observe, monitor, and report to the Nurse any change in condition.
   • f) Document observed data per organization policy.
   • g) Assist Elders with personal care functions.
   • h) Assist Elders with and/or perform restorative and rehabilitative procedures as outlined and assigned by the care plan and with appropriate training.
   • i) Measure and record vital signs such as temperature, pulse, respiration, weight, height, and anatomical dimension/circumference.
   • j) Assist moving, positioning, and transporting elders into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
   • k) Assist with the application of slings, elastic bandages, binders, etc.
   • l) Prepare and serve meals, between meal, bedtime and other nutritional snacks per the posted menu and the stated preferences of the Elders.
   • m) Keep all areas of the home clean, performing routine housekeeping duties.
   • n) Make occupied and unoccupied beds and change bed linens.
   • o) Launder clothing, linen and other articles.
   • p) Report accidents and incidents when they occur.
   • q) Perform other related duties as required by the elder.
   • r) Attend team meetings.
   • s) Attend and participate in Elder care conferences providing observations, achievements and concerns about Elder in your care.
5. Other Duties/Responsibilities
   • a) Maintain cleanliness of resident’s room including daily cleaning, deep cleaning as required, and discharge cleaning.
   • b) Assist Resident with care and handling of personal laundry.
   • c) Assist with dining including setting/cleaning tables, meal delivery [if necessary], serving meals, and washing dishes.
   • d) Provide information, encourage, and participate with residents in activity focused programs.
6. Physical Demands/Working Conditions
   • a) Able to sit, stand, bend, lift, push, pull, stoop, walk, and move frequently during working hours.
   • b) Able to lift 40 lbs. floor to waist, lift 10 lbs. waist to shoulder, lift and carry 40 lbs., and push/pull 40 lbs.
   • c) Able to assist in the evacuation of residents.

Qualifications7. Qualifications and Educational Requirements

• a) Must be able to effectively communicate in the English language and follow written and verbal instructions.
• b) High School Diploma or equivalent.
• c) Complete a minimum number of hours working as a CNA/STNA.
• d) Certified Nursing Aide Medication Technician [CNA-Med] or as required by Colorado regulations; pass a state board exam.
• e) CPR certification which must remain current during employment.
• f) First aid certification from a nationally recognized organization preferred.
• g) Minimum of one year experience in a senior care or healthcare setting preferred.
