Description: The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others. The nurse delivers patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards. The RN assesses and evaluates patient care needs and applies critical thinking skills in patient care management. They integrate relevant assessment and intervention skills in the delivery of nursing care. The nurse collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care. Prioritizing all aspects of patient care is essential, including teaching, rounding, coaching, and planning after hospital care while delegating to others as appropriate. Educating patients and caregivers while anticipating needs and readiness to learn about their plan of care is a critical responsibility. Effective and professional communication with patients, families, and all members of the Interdisciplinary Patient Care Team is crucial. The RN manages assignments within the Care Delivery Model of Modified Primary Nursing and demonstrates effective allocation of material resources. The nurse role models the tenets of Relationship Centered Care through various patient care facilitation actions. They consider the patient's values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care. Establishing and maintaining a therapeutic relationship with the patient and family is a priority. Using evidence-based practices increases understanding of patients' perceptions of care. The nurse initiates service recovery for patients and family members who have concerns and escalates to management as needed. Demonstrating caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions is vital. Supporting staff use of evidence-based practices to increase understanding of patients' perceptions of care is also part of the role. Conducting Purposeful Hourly Rounding on assigned patients addressing key needs is required. The Clinical Nurse practices safe and efficient patient-centered care while identifying and escalating barriers and opportunities for improvement. They demonstrate clinical skills and knowledge crucial to quality and safety in the patient population served. Implementing process improvement strategies based on institutional and evidence-based procedures is expected. Engaging in formal and informal peer and institutional review processes is required. The nurse collaborates with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities, and nursing research studies. Contributing to quality and safety practices and required compliance measures is essential. The nurse implements principles of high reliability to identify and avoid high-risk behaviors to provide a safe environment for patients. They escalate potential safety hazards or gaps from best practice following institutional protocols. Utilizing institutional information technology by documenting nursing practice to support quality and performance improvement initiatives is part of the role. The Clinical Nurse may perform charge nurse duties as assigned. Coordinating with clinical leadership to ensure supplies are properly utilized and inventory is sufficient for patient care is part of their responsibilities. The nurse escalates material or staffing resource deficits to clinical leadership. Demonstrating patient-focused and cost-effective approaches to patient care in terms of equipment, staff, supplies, and all other resources is crucial. Accurate and thorough documentation in compliance with regulatory and legal requirements is required. Facilitating patient throughput on a shift basis is an essential function of the role. The Clinical Nurse contributes to the environment of care to support clinical colleagues, patients, and their families. Advancing clinical competence in nursing practice to progress from novice to expert is encouraged. Projecting a professional image to colleagues and communicating with styles that demonstrate caring, respect, compassion, and empathy is crucial. Incorporating ethical principles into decision-making for patient and family is expected. Encouraging and demonstrating a spirit of scholarship, inquiry, life-long learning, and innovation for self and others is vital. The RN acquires knowledge and skills relative to the role, patient population, clinical specialty, and local health community needs. Participating in shared decision-making through specialty practice councils, nursing department committees, and unit initiatives is part of the role.
