Outpatient Physical Therapist

Avera Health, a regional health system based in Sioux Falls, SD, is seeking a compassionate and patient-centered Outpatient Physical Therapist to join our Healthcare & Medical Services department. In this role, you will provide evidence-based physical therapy services to patients in an outpatient clinic setting, helping them improve mobility, reduce pain, restore function, and enhance overall quality of life.

As an Outpatient Physical Therapist with Avera Health, you will evaluate patients with a wide range of musculoskeletal, neurological, and post-surgical conditions, develop individualized plans of care, and deliver therapeutic interventions that align with current clinical best practices. You will perform comprehensive examinations, including strength, range of motion, balance, gait, and functional assessments, and use these findings to establish measurable treatment goals in collaboration with patients, families, and the wider care team.

You will provide manual therapy, therapeutic exercise, neuromuscular re-education, balance and gait training, modalities as appropriate, and patient education focused on self-management and injury prevention. Regularly tracking progress, you will adjust treatment plans as needed and ensure accurate, timely documentation in the electronic medical record in accordance with regulatory and payer requirements.

This position is part of a collaborative outpatient care team that may include physicians, advanced practice providers, other therapists, nurses, and support staff. You will participate in interdisciplinary communication, coordinate care and referrals, and contribute to discharge planning and home exercise programming to support long-term patient success. You may also mentor students or new staff as appropriate and participate in quality improvement initiatives.
