Role Overview:

As our Registered Nurse, you are the clinical backbone of our home care operations. Because you are often the primary medical professional on-site, strong clinical judgment and independent decision-making are critical.

You will conduct comprehensive in-home client assessments, establish and update personalized care plans, perform caregiver competency evaluations, and provide ongoing supervision to ensure our caregivers deliver dignified, first-class care.

Key Responsibilities:

• In-Home Nursing Assessments: Perform initial and routine comprehensive health, functional, and environmental safety assessments in client homes across Baltimore.
• Care Planning: Develop, monitor, and adjust individualized person-centered care plans in compliance with Maryland Medicaid waiver standards.
• Caregiver Leadership & Supervision: Conduct in-person supervisory visits, evaluate caregiver competency, provide hands-on training, and mentor our caregiving team.
• Clinical Monitoring: Track client wellness, disease management, and changes in condition to prevent avoidable ER visits and hospital admissions.
• Advocacy & Communication: Act as a compassionate liaison and educator for clients and their families, ensuring clear communication and exceptional service.

Qualifications & Requirements:

• Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Maryland (Required).
• Clinical Experience: Proven RN experience with solid clinical assessment skills. Prior experience in home health, hospice, community health, or geriatric nursing is strongly preferred.
• Autonomous Decision-Making: Strong critical thinking and confidence operating independently in a field-based setting.
• Transportation: Valid Maryland Driver’s License, active auto insurance, and reliable personal vehicle for local travel within Baltimore and surrounding communities.
• Soft Skills: Warm, empathetic, and professional demeanor with a genuine passion for serving Medicaid and vulnerable populations.
• Certifications: CPR Instructor certification is a plus (or willingness to obtain).

• Pre-employment Drug Screen
• Negative Tuberculosis (TB) Screen / PPD
• Standard Physical Examination showing clearance for field nursing duties
