Position Overview RN Homecare Team Leader Job Summary: Coordinate, implement and continually evaluate the home care needs of patients. Oversee patient insurance, care plans, and ensure findings are shared with families and professional caregivers. Salary: $125,000 - $140,000 per year. Review patient insurance information and coordinate activities for each individual care plan, ensuring pertinent findings are shared with family and professional caregivers. Precept new nurses and supervise peers on field visits. Create the initial comprehensive, interdisciplinary patient‑centered home health plan of care; update plan in collaboration with physicians. Case‑manage care and services for assigned patients, achieving positive clinical outcomes and minimizing risks of rehospitalization. Accurately complete all OASIS data sets for correct reimbursement. Enter physician orders into EMR on the same day they are received. Provide accurate documentation to request authorization of services from MCOs, supporting continuity of care and reimbursement. Monitor and provide leadership and direction to field nurses, conducting supervisory visits in the field. Monitor and evaluate nursing documentation quality per policy, procedure, federal and state regulations. Education & Experience Education: Graduate from an accredited School of Nursing (BSN preferred).
